TO THE DEPARTMENT OF HEALTH AND HUMAN SERVICES — INDIAN
HEALTH SERVICE
WHEREAS, the Citizen Potawatomi Nation is a federally recognized tribe of American
Indians with constitutional authority authorized under the Oklahoma Indian
Welfare Act of June 26, 1936, (49 Stat. 1967); and
WHEREAS, the Citizen Potawatomi Nation has, through a history of organized self-
government since time immemorial, sovereign powers inherent in Tribal
tradition and recognized by treaties with the United States and in the United
States Constitution; and
WHEREAS, the Citizen Potawatomi Nation Constitution and By-Laws provide that
according to Article 7, Section 2, "Subject to any limitations in this
Constitution, and except for those powers expressly reserved to the Citizen
Potawatomi Nation Indian Council by this Constitution, or delegated to another
entity by this Constitution, the Business Committee of the Citizen Potawatomi
Nation is empowered to enact legislation, transact business, and otherwise
speak and act on behalf of the Citizen Potawatomi Nation in all matters on
which the Nation is empowered to act now or in the future", except those
relating to trust fund expenditures, trust land indebtedness and claims or
treaties with the United States; and
WHEREAS, the Citizen Potawatomi Nation has been awarded supplemental diabetes funds
in the amount of $401,022 from the Indian Health Service to enhance existing
diabetes services; and
WHEREAS, the Citizen Potawatomi Nation currently provides a comprehensive diabetic
education program and primary, secondary, and tertiary diabetic services to
American Indians within the Nation's service area including a specialized foot
clinic, insulin aids and magnifiers, pharmaceuticals, eye examinations, and
dental exams; and

WHEREAS, the Citizen Potawatomi Nation will receive full indirect costs in the amount of
$99,275.00; and
WHEREAS, the Citizen Potawatomi Nation is not required to provide matching or in-kind
donations to receive the supplemental funds; and
WHEREAS, the supplemental diabetes funds are for fiscal year 2001 and can be carried
over into fiscal year 2002.
NOW, THEREFORE BE IT RESOLVED that the Citizen Potawatomi Nation hereby
authorizes the submittal of the Supplemental Diabetes Grant Application for Fiscal Year
2001 to the Department of Health and Human Services — Indian Health Service.
NOW, THEREFORE, BE IT FURTHER RESOLVED, that by act of this resolution and
consistent with the separation of powers defined in the Constitution, the Business Committee
of the Citizen Potawatomi Nation hereby authorizes the appropriation of $401,022.00 from
the Diabetes fund or account for the negotiation, operation, enforcement, procurement,
settlement, and or completion of this grant agreement by the Tribal Chairman or his
designate.

Affairs and incorporated into this resolution by the attached July 2001 20-year
Transportation Plan Summary; and
WHEREAS, the Citizen Potawatomi Nation, through extensive transportation planning, has identified
an additional 43.28 road miles which are eligible for inclusion on the Bureau of Indian
Affairs Indian Reservation Roads (IRR) inventory and incorporated into this resolution
by the attached July 2001 20-year Transportation Plan Summary; and

WHEREAS, the criteria used to select and evaluate these roads include (1) connects active centers of
population, (2) contributes to industrial activities, (3) contributes to economic
development, (4) provides access to jobs, (5) contributes to law and order, (6) removes
isolation, (7) provides access to education and cultural activities, (8) provides access to
hospital facilities, (9) contributes to accident prevention, and (10) provides for emergency
services; and
WHEREAS, all of the proposed roads will either provide access to tribal services or have four or more
Indian families living along the roadway which impedes access to minor and major road
arteries leading to places of employment, health facilities, educational training, etc. all
provided at the Citizen Potawatomi Nation Tribal Headquarters; and
WHEREAS, all of the proposed roads miles are either on tribal trust land or adjacent to individual and
tribal trust lands; and
WHEREAS, this resolution amends Resolution Pott# 01-107 entitled, "A resolution supporting the
addition of new road miles to the Nation's existing Indian reservation road inventory and
submittal of the inventory to the Bureau of Indian Affairs" in order to increase the
number of reported existing road miles from 45.75 to 56.1 and request for new road miles
from 22.58 to 43.28;
NOW, THEREFORE BE IT RESOLVED, that the Business Committee of the Citizen Potawatomi
Nation supports the addition of new road miles to the Nation's existing road inventory and requests the
Bureau of Indian Affairs to add all or a large portion of the requested new road miles to the Nation's
Indian Reservation Roads Inventory and hereby amends resolution Pott# 01-107.
NOW, THEREFORE, BE IT FURTHER RESOLVED, that by act of this resolution and consistent
with the Constitutional separation of powers defined in the Constitution, the Business Committee of the
Citizen Potawatomi Nation hereby authorizes the appropriation of $3,390,335 from the BIA Roads

program services and provide five services: 1) information to caregivers about available
services, 2) assist caregivers in gaining access to the services, 3) individual counseling,
organization of support groups and training to caregivers to assist them in making
decisions and solving problems relating to their caregiver roles, 4) respite care to enable
caregivers to be temporarily relieved from their caregiving responsibilities, and 5)
supplemental services, on a limited basis, to complement the care provided by caregivers;
and

WHEREAS, there will be three program participant eligibility requirements: 1) Indian elders
raising grandchildren up to 18 years (as guardians), unless the child is mentally or
physically handicapped, 2) Indian elders caring for an elderly spouse, and 3) a non-Indian
caring for an elder Indian; and
WHEREAS, the supplemental funds will be awarded annually for up to four years.
NOW, THEREFORE BE IT RESOLVED, that the Business Committee of the Citizen
Potawatomi Nation supports the Title VI — Native American Caregiver Support Program
supplemental grant application and its submittal to the Administration on Aging.
NOW, THEREFORE, BE IT FURTHER RESOLVED, that by act of this resolution and

WHEREAS, the Citizen Potawatomi Nation opposes the idea of the ICDBG program becoming
nationally administered and scored because of disparities in reservation versus
non-reservation tribes, in particular, tribal needs and populations, which differ
significantly for Oklahoma tribes; and
WHEREAS, the Citizen Potawatomi Nation opposes the ICDBG program funding
announcement in the SuperNofa due to difficulties in finding grant guidelines in
over 700 pages and with relevant ICDBG scoring information being located in
multiple places; and
WHEREAS, the Citizen Potawatomi Nation opposes the ICDBG program scoring criteria for
economic development since it is now weighed upon benefiting the neediest as
opposed to creating the highest number of jobs and spin-off benefits.
NOW, THEREFORE BE IT RESOLVED, that the Business Committee of the Citizen
Potawatomi Nation supports the Indian Community Development Block Grant (ICDBG)
program and the continuation of the Southern Plains Regional Office of Native American
Programs (SPONAP) as it was established prior to the Fiscal Year 2001 SuperNofa changes.

A RESOLUTION APPROVING AMENDMENT 1 TO DIGITAL INSIGHT MASTER SERVICES
AGREEMENT AND EXHIBITS ATTACHED THERETO, ON BEHALF OF THE CITIZEN
POTAWATOMI NATION AND AUTHORIZING THE CHAIRMAN TO SIGN SAID AGREEMENT
WHEREAS, the Citizen Potawatomi Nation is a federally recognized tribe of American Indians with
constitutional authority under the Thomas-Rogers Oklahoma Indian Welfare Act of June 26,
1936, (49 Stat. 1967); and
WHEREAS, the Citizen Potawatomi Nation, the largest of the Potawatomi Indian Tribes, has, through a continuation of Potawatomi history and organized self government since time immemorial,
sovereign powers inherent in tribal tradition and recognized by treaties with the United
States and in the United States Constitution; and
WHEREAS, the Citizen Potawatomi Nation Constitution and By-Laws provide that according to Article
7, Section 2, "Subject to any limitations in this Constitution, and except for those powers
expressly reserved to the Citizen Potawatomi Nation Indian Council by this Constitution, or
delegated to another entity by this Constitution, the Business Committee of the Citizen
Potawatomi Nation is empowered to enact legislation, transact business, and otherwise
speak and act on behalf of the Citizen Potawatomi Nation in all matters on which the
Nation is empowered to act now or in the future," except those relating to trust fund
expenditures, trust land indebtedness, and claims or treaties with the United States; and
WHEREAS, the First National Bank has requested the Citizen Potawatomi Nation to enter into a Master
Services Agreement (LT) with Digital Insight on its behalf; and
WHEREAS, First National Bank will reimburse the Citizen Potawatomi Nation for all costs of this
transaction; and
WHEREAS, First National Bank does hereby sell, transfer, assign and set over to the Citizen
Potawatomi Nation all of its rights, title and interests in and to said Agreement;
NOW THEREFORE, BE IT RESOLVED that the Business Committee of the Citizen Potawatomi Nation
hereby approves the attached Amendment 1 to Digital Insight Master Services Agreement (LT) and the
exhibits attached thereto and authorizes Chairman John A. Barrett, Jr. to sign said agreement and exhibits on behalf of the tribe.

NOW, THEREFORE, BE IT FURTHER RESOLVED, that by act of this resolution and consistent with
the Constitutional separation of powers defined in the Constitution, the Business Committee of the
Citizen Potawatomi Nation hereby authorizes the appropriation of $22,700 from the General Account
fund or account for the negotiation, operation, enforcement, procurement, settlement, and or completion of this contract or agreement by the Tribal Chairman or his designate.
